Today’s post-acute team is far more extensive and integrated than ever before.  The unique roles and responsibilities provided by members of the post-acute care team and cohesive, coordinated efforts is essential in helping to prevent hospital readmissions and enhance the overall quality of care.  This seminar kicks off with an interdisciplinary led panel representing various members of the post-acute team, highlighting the importance of effective collaboration to improve efficiencies and wound care outcomes.

Successful wound management beings with a thorough assessment of the wound and periwound skin.  To determine an appropriate treatment plan and monitor progress over time, wound and skin assessment needs to be comprehensive, systematic and evidence based.  Incorporating a case-based scenarios and interactive polling, this session will explore critical elements for skin and wound assessment.
